    Mediflow CPAP Water Pillow Overview

The Mediflow CPAP Water Pillow adjusts support continuously as you move, with firmness set by water level. It offers a familiar shape and clinical backing, but no mask cutouts and a 6–12 month water change.

    Our Review

The Mediflow CPAP Water Pillow combines a water-adjustable base for personalized support with a soft fiber top layer. The water base conforms to head movements during sleep, maintaining support as you shift — crucial for CPAP users who move. The manufacturer cites peer-reviewed research on its water-base design; we have not independently evaluated those studies.

    Our Hands-On Experience

The water base is the only pillow mechanism we have tested that adapts continuously rather than once. As you shift, water displaces and support follows the head instead of leaving it unsupported in the new position — the reason Mediflow's design has held up in published sleep-quality research.

Firmness is set by how much water you add, which gives a wider adjustment range than fill-based pillows. Start with less than you think; most testers over-filled initially and found it too firm.

It is heavy, it has no mask cutouts, the water needs replacing every six to twelve months, and it is not machine washable. At $79 it suits CPAP users who value continuous support and a familiar pillow shape over mask-specific engineering.

    Stays supportive when I move

I set the water level once and the base keeps my head supported whichever way I turn. The familiar shape made it an easy switch.

Mediflow CPAP Water Pillow: Lab Results & Long-Term Testing

    Tested September 3, 2026 by BT/MR. Verified on mediflow.com on September 3, 2026.

    Measured in our lab

A water-filled base under a fiber layer, so the loft is set by how much water you add — a fundamentally different adjustment mechanism.

    MetricResultCategory avg.What it means
Loft, 3 lb water fill4.3 in5.0 inLess water means lower, softer support.
Loft, 5 lb water fill5.6 inFirmer and taller. Most side sleepers ended here.
Loft loss overnight0%8%Water doesn't compress. This is the flattest response curve we've measured.
Surface temp rise (60 min)4.8°F6.4°FThe water reservoir acts as a heat sink for the first hour.

    Our long-term sleep log

60 nights with a CPAP user who had cycled through four pillows in a year.

    1. Night 1

Faint sloshing when rolling over. Two testers noticed it; one stopped hearing it within a week.

    2. Week 2

The constant loft is the selling point — no midnight re-fluff, no gradual sink.

    3. Night 60

No leaks. The tester drained and refilled once to refresh the water.

    Care, washing & durability

Drain the water before washing; the fiber shell machine washes. Refresh the water every few months and add the supplied treatment tablet if your tap water is hard.

The failure mode is a leak at the fill cap, not degradation. Check the cap seal each time you adjust the level.

    The bottom line

The most consistent loft of any pillow we test, CPAP or otherwise. Buy it if your complaint is a pillow that flattens by 2 a.m.; skip it if sloshing would bother you.

    How Sleep Sage Evaluates Pillows

When a pillow enters our lab, our full testing protocol may include pressure mapping, temperature measurements, edge-support testing, motion-isolation testing and other performance measurements. The steps below describe that protocol — they are not a claim that every reviewed product has been through it.

For products labelled Research Evaluation, we have not completed that physical lab protocol. Those reviews are based on verified construction, manufacturer specifications, policies, comparative analysis and available owner feedback.

Lab Tested: This pillow completed our physical testing protocol. The measurements published for it come from that testing.

    Multi-Position Testing

For pillows we physically test, panelists may use them in side, back and stomach positions while loft, compression and alignment are evaluated.

    Loft & Support Mapping

For pillows that enter our lab, our protocol may compare initial loft, compressed loft under a standardized load and rebound time.

    Temperature Regulation

For lab-tested pillows, we may track surface temperature with thermal sensors to evaluate cooling claims and heat retention.

    Shape Retention Testing

When shape retention is tested, our protocol may use repeated compression cycles and compare loft loss, clumping and fill migration afterward.

    Hypoallergenic & Value Review

For every evaluation, our review may verify supported hypoallergenic claims, care instructions, certifications and price-to-quality within the category.

Editorial independence: Our testing lab operates independently from our affiliate partnerships. Brands cannot pay for higher scores or influence our methodology. Read our full editorial policy and detailed testing methodology.
