Best CPAP Pillows for Nasal Masks

A nasal mask covers the nose only, so it carries less bulk than a full-face frame but more than a nasal pillow mask. It is the middle case: the cushion and frame still meet the pillow when you turn onto your side, and the straps that hold it are easy for a pillow edge to shift.

These CPAP pillows carry a research evaluation: rankings are based on shape, loft, mask clearance, materials and verified manufacturer specifications, together with long-run owner feedback. We do not measure mask leak rates, AHI or therapy adherence.

What had to be true to qualify

  • The manufacturer documents nasal-mask compatibility. Pillows whose makers say nothing about nasal masks are excluded rather than assumed to fit.
  • Currently sold in the US with a stated return window.
  • Already in our canonical pillow database — 5 CPAP pillows were considered, 5 qualified.

Ranked by cutout depth first, then side- and back-sleeping suitability and adjustable loft, with the pillow's overall database score breaking ties. Specifications verified September 14, 2026.

What a nasal mask actually needs from a pillow

Two things: somewhere for the frame to sit, and nothing pushing the side straps. A nasal mask is held by headgear that runs across the cheeks, so a tall pillow edge can lever the cushion off the nose even when the frame itself is clear.

That is why a shallow decorative contour tends to disappoint here. You want an opening wide enough that your cheek rests on the pillow and the mask does not.

Loft matters as much as the cutout

A nasal mask sits on a mobile part of the face, and head angle changes the seal geometry. A pillow that is too tall tips the chin towards the chest and rotates the cushion; too low and the head drops away from it.

Adjustable-fill or insert-based pillows are useful for exactly this reason — you can correct height in small steps instead of buying again.

The pillow is one variable among several

Cushion size, cushion age, headgear tension and facial hair all affect a nasal seal, and none of them is something a pillow changes. If your mask moves without the pillow touching it, the pillow is not the thing to replace, and a persistent leak is worth raising with your equipment provider.

How we ranked these pillows

Documented compatibility gates the list. A nasal mask is small enough that most pillows would probably cope, but 'probably' is not a specification, so we rank only pillows whose maker states nasal-mask fit and we quote that statement rather than paraphrasing it into a promise.

Among the pillows that qualify, cutout depth still leads, because a nasal frame plus its straps is exactly the kind of low-profile hardware a shallow contour fails to clear. Position suitability comes next, and adjustable loft breaks near-ties because height is the thing most nasal-mask users end up correcting.

These are research evaluations. We compare published shape, cutout design, loft, adjustability, material, cover and hose features against manufacturer specifications and long-run owner feedback. We publish no leak rates, AHI figures or adherence statistics, because we have not measured them.
