    Best CPAP Pillows of 2026

A CPAP pillow is shaped to leave room for the mask and hose. The right one keeps the pillow from pressing against the mask when you turn onto your side, without giving up neck support or comfort.

Who should consider a CPAP pillow?

A dedicated CPAP pillow is most useful if you sleep on your side, wear a bulkier full-face mask, notice the mask contacting the pillow, reposition often during the night, or want a built-in place to route the hose. If none of those describe you, an ordinary supportive pillow may be all you need — the longer version of that question is in CPAP pillow vs regular pillow.

Best CPAP pillow by mask type

    Full-face masks

Full-face frames cover the nose and mouth and project farther from the face, so they are the mask style most likely to touch a pillow when you roll onto your side. A deeper side cutout or a pronounced mask-clearance contour gives the frame somewhere to go instead of being pushed against your cheek.

    Nasal masks

Nasal masks are smaller, but they can still shift when your face presses into a pillow — the cushion sits right where a side sleeper's cheek lands. A moderate cutout is usually enough, and loft matters as much as the cutout itself.

    Nasal pillow masks

With very little mask bulk, deep cutouts matter less. Most users in this group do better prioritising comfort, loft and sleeping position, and a well-chosen ordinary pillow is often fine.

Mask fit and sizing are questions for your equipment provider or clinician; the notes above are about pillow shape only.

Best CPAP pillow by sleeping position

    Side sleepers

This is where the category earns its keep. Look for a genuine side cutout, enough loft to fill the gap between your ear and the shoulder you are lying on, and a firmness that holds that height rather than collapsing. A pillow that is too soft lets the head sink until the mask meets the pillow anyway.

    Back sleepers

Mask contact is less of an issue lying face-up, so the priority is neutral head and neck positioning without excessive elevation. A contoured pillow that keeps the head from rolling to one side can help the mask stay where you set it.

    Combination sleepers

If you move between positions, the useful features are cutouts on both sides, a responsive material that recovers quickly, and adjustable loft so one pillow is workable in more than one position. Hose routing matters more here too, because turning is when the hose tugs.

Do you need a special pillow for CPAP?

Not everyone does. Plenty of CPAP users sleep well on an ordinary pillow, particularly back sleepers and people wearing nasal pillow masks. A contoured CPAP pillow tends to be more useful if you sleep on your side, use a bulkier full-face mask, notice the mask contacting the pillow, reposition frequently, or want dedicated hose management.

If your current pillow already keeps your head supported and your mask undisturbed, there is no reason to replace it. Full comparison: CPAP pillow vs regular pillow.

How to choose a CPAP pillow

    • Sleeping position — side sleepers need more loft and a real cutout; back sleepers need less of both.
    • Mask type and clearance — the bulkier the frame, the deeper the cutout should be.
    • Loft and adjustable fill — adjustable pillows let you correct height without a second purchase.
    • Firmness and material — foam holds shape at the cutouts; shredded or latex fills are easier to reshape.
    • Cooling and cover — memory foam runs warmer; a removable, washable cover is worth having with a mask in contact nightly.
    • Size and hose routing — check the pillow fits your pillowcase, and whether a hose channel or tether suits how your bed is set up.
    • Return policy — an unusual shape takes a few nights to judge, so a trial period matters more than usual here.

    How we evaluate CPAP pillows

CPAP pillows on this page carry a research evaluation. We compare published shape, cutout design, loft and adjustability, material, cover and hose features against manufacturer specifications and long-run owner feedback, and we score them on comfort, support and value in the same way as the rest of our pillow database. We do not publish leak rates, AHI figures or adherence statistics, because we have not measured them — those are clinical measures, and a pillow is a comfort and fit accessory.

    Mask leaks and side sleeping

If your mask shifts or leaks when you turn over, pillow shape is one possible factor among several — strap tension, cushion wear, mask sizing and hose drag all play a part. Our troubleshooting guide walks through the non-prescription checks: CPAP mask leaks while side sleeping, and how to sleep on your side with CPAP covers positioning and hose routing.
