These pillows are still in our main CPAP pillow ranking — they simply do not fit this use case.
Side sleeping needs enough loft to fill the gap between your ear and the mattress over your shoulder. Back sleeping needs considerably less. One pillow cannot be both heights, so combination sleepers end up slightly high on the back or slightly low on the side.
Err towards the side setting if most of your night is spent there, and towards the lower setting if you fall asleep on your back. A pillow with removable fill or inserts lets you move that line by small amounts instead of guessing once.
The moment of turning is the mechanically awkward one: the mask sweeps across the pillow surface, the headgear catches, and the hose crosses your body. A cutout on both sides means the frame has somewhere to land on either turn instead of being levered by a flat edge.
Hose slack above the head matters more for you than for a still sleeper, because your tubing is repeatedly dragged from one side to the other. A tether, a hose lift or simply routing the tube over the headboard removes most of that pull.
Judge a new pillow on whether the mask is where you left it in the morning, and on where you feel stiff — not on whether the shape felt familiar on night one. If one position is clearly worse, adjust height before assuming the pillow is wrong.
Anything persistent about the seal itself belongs with your equipment provider. A pillow only changes what the pillow does to the mask.
The ranking is scored on a pillow's weakest position rather than its best. That is the whole point of this page: averaging an excellent side score against a below-average back score would recommend a pillow that fails you for part of every night.
