Why hose routing outranks mask clearance here

Nasal pillow masks have a short frame and a light headgear, which is what makes them comfortable — and also what makes them easy to displace. The hose is the heaviest thing attached to your face, and every time it drags across the bed it pulls the cushions sideways in their nostrils.

Getting slack above your head, through a pillow tether, over the headboard or on a hose lift, removes that pull. A pillow with a built-in channel or tether does part of the job for you.

Height is the other half of the fit

A tall pillow tips the chin down and changes the angle at which the cushions meet the nostrils; a flat one lets the head roll back. Neither is dramatic, but with this mask style the seal is small, so small angle changes are felt.

Aim for your head sitting roughly in line with your spine in whichever position you sleep, and prefer a pillow firm enough to hold that height all night.

You may not need a CPAP-shaped pillow at all
