    1. The mattress: medium-firm and durable

    Support and durability carry the weight here. A bed that scores well on comfort but poorly on durability will develop a hollow at the hips, and that hollow is what people describe as their mattress causing back pain.

    2. The base: the part people skip

    A slatted platform with gaps over three inches, or a foundation with a broken centre support, lets even a firm mattress dip. An adjustable base additionally lets you raise the knees or head, which some people find takes load off the lower back.

    Your neck is the top of the same column. A pillow that is too tall in side sleeping, or too tall in back sleeping, tilts the head and the strain travels down.

    Check the base before you replace anything

    Strip the bed and look underneath. Slats more than three inches apart, a bowed centre rail, a missing centre leg on a queen or king, or a box spring older than the mattress are all common and all cheap to fix relative to a new mattress.

    If the mattress sits on the floor or on a solid platform and you can still see or measure a hollow where you sleep, the mattress itself has failed, and that may be a warranty claim rather than a purchase.

    Firmness is not the same as support

    A firm mattress can be poorly supporting, and a medium mattress can be strongly supporting. Firmness is the surface feel in the first inch; support is whether the core holds your hips level with your shoulders. Our support rating is scored separately from firmness for exactly this reason.

    For most people with lower back pain, the useful window is medium-firm — around 6 to 7 out of 10 — with heavier bodies landing at the top of that range and lighter bodies at the bottom.

    Is a firm mattress best for back pain?

    Not automatically. Research and our own testing both point to medium-firm rather than firm for most people, because a very firm surface leaves a gap under the lumbar curve for side sleepers. What matters is that your hips do not sink lower than your shoulders.

    Can a topper fix a mattress that hurts my back?

    It can fix a mattress that is too soft, and it cannot fix a mattress that has sagged. If you can lay a straight edge across the bed and measure a gap where you sleep, a topper will follow that dip rather than bridge it.

    Does an adjustable base help back pain?

    Some people find that raising the knees slightly reduces lower-back load, and raising the head helps with reflux-related discomfort. It is a comfort adjustment rather than a treatment, and it works only if the mattress on top of it is already supporting you.

    How long before a new setup stops hurting?

    Give a new mattress about 30 days before judging it — your body adapts, and foams break in. That is also why buying inside a trial period with a real return path matters more here than in most categories.
